As the PSNC urges pharmacies to embrace new services NICE urges commissioners

to exclude medicines from PGDs often delivered by pharmacist.Pharmacists who

use PGDs should stand up for them or they will lose out on services which we were

encouraged to deliver? Practice commissioners have been asked to consider alternatives

such as independent prescribing?

The Chief Medical Officer claims that antimicrobial resistance poses a “catastrophic threat”

to public health and Nice has called for a stop to “Unnecessary and inappropriate” PGDs

and their recommendations are open for consultation until April 29thand new guidelines

are expected in June. Nice has asked for medication such as antibiotics, medication for

long term conditions such as diabetes , drugs that require complex monitoring or variable

doses and black triangle medicines to be excluded. Nice believes that PGDs are appropriate

for some medication such as EHC and contraception. The NPA has removed all antibiotics

except azithromycin for chlamydia from their PGDs after consultation with the DH.

In times when Walgreens offer inoculations for 17 different conditions including shingles

and meningitis and want to roll this service out across Boots store in the UK we are to retract

PGD services if they include antibiotics?

American pharmacists run HIV excellence centres and are involved in giving

specialist advice on medication at stages of the disease process yet a pharmacist is not to

be trusted by nice not to over prescribe an anti-biotic under a PGD? It does not make sense.

Mid Staffordshire NHS Foundation Trust Public Inquiry Report made me think …..

Why does it take a group of patients and cares “CURE” under the lead

of Julie Bailey, to hold the Regulators and, The Government to account.

A plethora of health care professionals, were involved and many must

have been aware of the poor quality of care. Do you; turn a blind eye to, poor

standards in your practice?

The recent public enquiry was announce by The Rt Hon Andrew Lansley CBE MP,

as he felt it was a failure of the trust first and foremost but also a national failure

of the regulatory and supervisory system. The role of all the above is to

ensure quality, safe and effective patient care.

One previous report highlighted a “Culture of fear” which discouraged staff from

reporting concerns.

One of the enquiry’s terms of reference was

“To examine the operation of the commissioning, supervisory
and regulatory organisations and other agencies, including their monitoring
role “

One key recommendation was that all service providers, regulatory
and ancillary organisations in health care should consider the findings and
recommendations and look to their own practice.

Patients Cases were heard and reports such as

“Triage in A&E was undertaken by untrained staff”

were brought to light. How often do we have untrained staff working in

your practice, or they may be trained but they have no record of it?

What training records do you have? One of the previous report

highlighted that

“Management thinking during the period under review was
dominated by financial pressures and achieving FT status, to the detriment of
quality of care.”

Due to increasing pressure on the remuneration in pharmacy have
you, taken your focus, away from professional standards? When did we last rotate
stock or date check our stock?

How can you make it better and how do you audit, monitor and
improve your practice?

Have you completed a patient survey this year? Do you record complaints and incidents?

Do you investigate them and improve your SOPs? Look at your Pharmacy
standards, the culture in your pharmacy and, your professional practice, create a baseline and
set yourself goals to improve your standards.

If you are too busy, let Pharmacy Consulting help you.

We can help you with SOPs, Audit, and Training, preparing an
application for a warehouse licence (WL) or for a MHRA inspection of your WL?

If your pharmacy is wholesale dealing medicines for human use you may be potentially contributing to UK shortages and be in breach of both supply chain obligations and GPhC standards of conduct. (www.pharmacyregulaton.org) The Department of Health has issued new guideline which affects pharmacies and all the players in the supply of medicines, from MA holders to dispensing doctors. The Human Medicines Regulations 2012 (SI 2012/1916) “the 2012 Regulations” state that if you wholesale any medicinal products for human use, you are required to hold the necessary licence, unless an exemption applies. Pharmacies must always make patients their first concern and ensure that their professional judgement is not compromised by commercial interests, targets or organisational interests.

Pharmacies must obtain a wholesale dealers licence (WDL) if they want to carry on wholesale dealing. Pharmacies must also only buy medicines from WDL holders in the UK or EEA member states. Pharmacy Consulting Limited will help you to obtain a WDL for your business and can provide “Contract RP “services if required. If you would like any further advice please contact Jackie@pharmacyconsulting.co.uk

Pharmacy Consulting recently had a client who had had his 100 Contract Application put on hold because the PCT wanted to tender an out of hour’s contract to all contractors in the area. My client took the PCT to court and won his case. Pharmacy Consulting then applied for an out of hour’s contract for him and his application was commended for its standard and completeness. They did not grant the contract to him because none of the other applications met the standard! We are appealing this decision. Do you need help with an appeal against your PCT ? Or help with a tender contact jackie@pharmacyconsulting.co.uk

In your busy working day do you ever think about your Key Business Indicators (KBIs)? If not why not? Pharmacy consulting can help you determine these. You need to have a clear strategic plan which feeds into your business plan which then drills down into staff’s Key Performance Indicators (KPIs). All your objectives should follow the “KISS”, Keep It Simple Stupid, and “SMART”, Specific, Measurable, Achievable, Realistic and Timely principles.

Once all the KBIs and KPIs have been set then you need to be a good leader with clear vision to drive them, and optimise the outcomes. Pharmacy consulting can help you develop your leadership skills.

Many Pharmacy owners do focus on their financial business indicators such as

Sales

Expenses

Profits

Cash flow

Capital expenditure

Margins

But KBIs can be much more powerful and can show you where to focus your efforts to grow your business. If you have a new pharmacy you may need to focus on gaining new customers to obtain your practice allowance. An existing pharmacy may focus on retention and link selling, or they could be under threat from their nearest surgery moving further away . Internet pharmacies may be trying to minimise their delivery costs. All pharmacies will be trying to maximise the services they are trying to provide and all must keep their focus, customer centric and maximise their consent to service and nominations.
